Meal Plan Change Request Form

If you wish to change your Meal Plan for the Spring 2021 semester, please go ahead and complete your contract now. The final deadline for changes will be January 13, 2021. NO changes will be accepted after the deadline. For a detailed description of the Meal Plan options, please go to Meal Plans & Flex Dollars under the Dining Services tab.   • For their Fall semester, Freshmen living on campus are required to purchase the 21+ Meal Plan.
  • If you do not complete a new contract, you will remain on the same Meal Plan you have this semester.
  • If you were studying remotely or abroad for a semester, you will need to complete a new contract, otherwise you will be assigned the 21+ Meal Plan.
  • You DO NOT need to Log In to the Mount Card site to complete the contract. You do, however, need to include your Student ID number on the contract.
  • Meal plans can only be changed using the online contract, not by phone or email.
  • A confirmation email will be sent to the email you provide once your contract has been successfully submitted. Please keep this email for your records. If you do not receive the confirmation email, your contract changes did not go through and should be resubmitted.
  • The deadline for changes is January 13, 2021.
  • Please be sure to complete all fields and hit Submit.
